    School Year 2019-20 Sports Registration Packet Directions


    Dear Columbia Middle School Parents/Guardians & Students:


    I would like to extend a warm welcome to all new and returning Tigers for the 2019-2020 school year! 


    Many of you will be participating in interscholastic athletic program this year and your experiences will be most rewarding.  We are very proud of our athletic programs here at Columbia Middle School and we look forward to your participation. 


    In order to participate in athletics at Columbia Middle each student-athlete must meet the following requirements:

    We will continue to use the 3 step process.


    STEP #1- ALL required sports registration details will be completed on-line via a platform called Form ReLeaf.  It is important to complete this process in order to be on the list to play a sport during the 2019-2020 school year.


    FormReLeafis a web-based program allowing parents to complete forms ONCE, and then the information will be prepopulated IF you have more than one child OR your child plays more than one sport. 


    STEP #2- Medical forms still need be to be completed and returned to the Health Office at the Middle School- BY THE POSTED DUE DATE because of the requirement for the Berkeley Heights School District Doctor’s signature.  Please note that these forms must be printed and completed by an NJDOE Cardiac Module Certified Doctor. 


    STEP #3- Complete the Sports Activity Fee

    Log on to the site:   www.payforit.net

    All student athletes must submit a once per school year Activity Participation fee of $100.00.  

    Payments can be made online at the www.payforit.net website after June 1, 2019, for the 2019-2020 school year.  Please log onto the site and scroll down to the 3rdline:


    Middle School Athletics Fee for 2019-2020- $100.00. Click the big green + to complete the process.


    To register on the website you will need your student’s PowerSchool student ID number found by clicking into the Demographic Update page in the parent portal of PowerSchool.  Following Name (last, first MI) you will find the student ID number.

    If you have any questions, please do not hesitate to call or email the Health Office (sprendville@bhpsnj.org) and (908) 464-1600 ext. 1630.  Please note the nurse will not be in over the summer.


    Once a student athlete has cleared the 3 Step process- you will receive an email indicating your child has been cleared to participate in athletics.
