All students new to the Berkeley Heights School District must provide documentation
of a physical performed within the past year including complete immunization records within 30 days of their first day of school.
If the student requires medication at school, the doctor and parent must complete medication forms.
All medication must be delivered to school by the parent in its original container.
Pages 1 and 2 are required for all medication administration orders.
Pages 1, 2, and 3 are required for self-administration medication orders.
