Before entry into 6th grade, students are required to receive two immunizations; a 
    Tdap booster and meningococcal vaccine.
    Parents/guardians are required to provide the school with a documented record of immunization.  Official school records, health department records, and/or physician signature shall be accepted as immunization documentation.  The records must include the month, day, and year the immunizations were given.  Medical and religious exemptions may be granted with appropriate documentation.

    Meets Immunization Requirements
    Diphtheria, Tetanus, Pertussis
    DTap, Td, or DT 4-5 doses, having received the 4th dose on or after the 4th birthday OR any 5 doses
    (ages 7-9) 3 does of Td or any previous combination of DTP, DTap, and DT
    Grade 6 (or comparable age level for special education programs) 1 dose
    Polio Vaccine
    (Age 1-6 years) 3 doses, with one given on or after 4th birthday, OR any 4 doses
    (Age 7 or older) any 3 doses
    Either Inactivated Polio Vaccine (IVP) or Oral Polio (OPV) separately or in combination
    Diphtheria, Tetanus, Pertussis DPT) Vaccine DTap, Td, or DT
    4-5 doses, having received the 4th dose on or after the 4th birthday OR any 5 doses
    Polio Vaccine (OPV or IVP)
    3-4 doses, having received the 3rd dose on or after the 4th birthday OR any 4 doses
    MMR (Measles, Mumps, Rubella) Vaccine
    2 doses with the 1st dose given after the 1st birthday
    Hepatitis B Vaccine
    3 doses (Mandatory for grades K, 1, 6, 9-12) and all children transferring into a NJ school from out of state
    Haemophilius Influenzae Type B (Hib)
    Pre-school students must have received one dose
    Varicella (Chickenpox)
    Required for pre-school, K & grade 1
    Freshman & new transfer students at 4 year colleges living in on-campus dormitories
    Meningococcal (meningitis)
    Influenza (AGES 6-59 MONTHS) 1 dose ANNUALLY
    Child care, pre-school or pre-Kindergarten on or after 9/1/08
    One dose to be given between Sept 1 and Dec 31 of each y

    Union County Immunization Center offers free immunizations to children residing in Union County meeting the following criteria:
    (1)  Medicaid, Medicaid Managed Care, and NJ KidCare Plan A
    (2)  NJ KidCare Plans B,C, & D
    (3)  Has no health insurance
    (4)  Is an American Indian or Alaskan Native
