ImmunizationsBefore entry into 6th grade, students are required to receive two immunizations; a
Tdap booster and meningococcal vaccine.Parents/guardians are required to provide the school with a documented record of immunization. Official school records, health department records, and/or physician signature shall be accepted as immunization documentation. The records must include the month, day, and year the immunizations were given. Medical and religious exemptions may be granted with appropriate documentation.
