Medication Administration
All medication, whether prescription or over the counter, must be delivered to the Health Office in the original pharmacy container. In addition, a Administration of Medication Form must be completed and submitted to the Health Office each school year per New Jersey State and Berkeley Heights Board Public Schools regulations. Students who use Epipen or Asthma Inhalers may carry these medications and self-administer provided the proper forms have been completed and submitted to the Health Office. Please refer to the

Sports Participation
The new rules directed by NJ Statutes (N.J.S.A. 18A: 40 – 4) and New Jersey State Department Of Education/School Health Service Guidelines state that the private physician chosen by the parent must conduct the students’ medical examination, including all physicals required to participate in interscholastic sports. It has also been determined that the Berkeley Heights Public School physician may also perform these physicals. School physicals will be performed yearly during the first two weeks of June for all students interested in trying out for any Interscholastic sport who attend Columbia Middle School.
New Jersey Administrative Code 6A:16 have recently been updated. All students who wish to tryout for a school athletic team or squad must have a physical exam within 365 days prior to the first practice session. The sports physical must be completed in its entirety on the updated official New Jersey State Board of Education Sports Physical Form as of May 1st, 2010. Incomplete forms will not be accepted. ~Discard all old forms and download the forms from this website or obtain from the health office. PE Excuses
A student may be excused from physical education class by a parent note for a period no longer than three days. An excuse lasting more than three consecutive days must be submitted on a physician’s note specifically stating the diagnosis and the duration of exclusion from physical education. Students cannot resume gym activities after a serious illness or injury without a medical release from the student’s physician.
Students are to bring the parent notes directly to the gym  teacher and any doctor notes to the nurse at the beginning to their scheduled time block for physical education. New physician documentation must be submitted each school year for any restrictions or exclusions for the student’s physical education program.

Screenings:

Auditory Screening
The purpose of the school auditory screening program is to identify students with any hearing loss that may impact their intellectual, emotional, social, speech, or language development. All seventh grade students will be screened during the current school year.

Scoliosis Screening
According to new state guidelines, every student between the ages of 10-18 must be screened for scoliosis every other year. Scoliosis is a lateral curvature of the spine, most commonly detected during adolescent growth period. At the middle school all seventh grade students are screened for scoliosis during their Health/Physical Education class. The exact date is determined and announced to the students a few days before screenings. Any student may be excused from the scoliosis screening upon written request of the parent/guardian.   See link below.  Submit the form to the Health Office.  

A referral will be sent by mail for those students who need to be further evaluated by their private physician. Please be sure to return the referral form as soon as the physician has completed it.

Vision Screenings
In order to detect visual impairment, the vision screening process is essential for all children enrolled in the school. All students have their vision screening once during each school year.  Students who have difficulty passing these screening tests will be tested a second time. If the student still has difficulty passing the vision test a notice will be sent home for the parent/guardian to follow up with their own physician. Please be sure to return the referral form once it has been completed.

Physicals
According to new regulations from the New Jersey Department of education, the Berkeley Heights Public Schools are no longer requiring a physical but it is extremely important and we strongly urge you to obtain a medical examination at least one time during the adolescence stage (grades seven through twelve). This will be important if working papers need to be completed. We at the health office will not be able to sign working papers without a copy of the physical exam conducted by your own physician in our files.
Any parent/guardian who is unable to obtain a physical for their child by their private physician should contact the Health Office for assistance.

Vaccine Requirements
        Effective September 1, 2008, the New Jersey Department of Health and Senior Services (NJ DHSS) advised that the Governor had recently signed into law a legislative bill which  mandates 2 additional immunizations for all students entering sixth grade.

Diptheria and tetanus toxoids and pertussis vaccine ( 8:57-4.10):  

        Every child born on or after January 1, 1997, and end entering  ro attending the Grade Six, or a comparable age level special education program with an unassigned grade on or after September 1, 2008 shall receive one dose of Tdap (tetanus, diphtheria, acellular pertussis) given no earlier than the 10th birthday.
        Children entering or attending Grade sixth on or after September 1, 2008, who received a Td booster dose less that five years prior to entry or attendance shall not be required ro receive a Tdap dose until five years have elapsed from the last  DTP/DTaP or Td Dose.
         Children born on or after January 1, 1997, and transferring into a New Jersey school from another state or country after September 1, 2008, shall have received one dose of Tdap, provided at least five years have elapsed from the last documented Td dose.  

Meningococcal vaccine (8:57-4.20):
        Every child born on or after January 1, 1997, and entering or attending Grade Six,  or a comparable age level special education program with an unassigned grade on or after September 1, 2008 shall receive one dose of a meningococcal-containing vaccine, such as the medically-preferred meningococcal conjugate vaccine.
 *Please note: This applies to students when they turn 11 years of age and attend Grade Six.
        Every child born on or after January 1, 1997,and transferring into a New Jersey school from another state or country after September 1, 2008, shall have received one dose of meningococcal vaccine.
*Records that are submitted must be documented by a physician signature, and must include the month, day, and year that the immunization was given.
