• For any student requesting a sports physical in school, the Complete Packet is to be submitted to the Health Office by Friday, May 2, 2013. 
• For any student who is planning to participate in a fall sport and is not requesting a physical in school, pages 1 through 11 need to be completed and submitted to the Health Office by Friday, May 24th.
